*	請於2023年2月23日(星期四)或之前以電郵 (skiplunch@commchest.org) 或傳真(2506 1201)確認所需愛心券數量。愛心券數量有限，以先到先得方式處理，送完即止。所需愛心券數量一經確認，將不能更改、取消或退回。
	Please confirm the quantity of Skip Lunch Day Coupons via email (skiplunch@commchest.org) or fax (2506 1201) on or before 23 February 2023 (Thursday). Coupons are available on a first-come, first-served basis while stocks last. Once the quantity of Coupons is confirmed, NO change or cancellation or return will be accepted.

*	公益金將以電郵/電話確認收妥貴機構的回條，請於收到公益金的確認後，方把善款存入公益金之戶口或把抬頭書寫「香港公益金」的劃線支票寄回公益金辦事處。公益金將於2023年3月初通知各機構有關愛心券之安排。
	The Chest office will send a confirmation to the participating organisations by email / telephone upon receiving the Reply Form. After receiving confirmation from the Chest, please deposit your donation into the Chest’s account or make cheque payable to “The Community Chest of Hong Kong” and send it to the Chest office. The Chest office will notify the participating organisations on the arrangement of Coupons by early March 2023.

若貴機構不同意公益金使用上述資料與貴機構保持聯絡，作定期介紹公益金的各項籌款活動、宣傳及推廣事宜、定期通訊、義工服務及意見收集之用，請於方格內加上✓號。
If your Company DOES NOT WISH The Community Chest to use the data collected to keep your Company updated with the Chest’s fund-raising events, marketing & promotional activities, newsletters, volunteer services and surveys to collect donor opinions, please tick the box.
